The near point of the eye is the point where the eye can see clearly without having to strain them which is 25 cm. This limit comes from the lens present in the eye which cannot focus the light on the retina if the object is closer than 25 cm.
If the object we want to see is closer to the eye than this distance, we can still see it but we would have to strain our eye such that the image of the object is formed on the retina clearly. Hence the ciliary muscles would change the shape of the lens to change its focal length and hence we would have to strain our eyes.
The far point of the eye is the farthest distance at which we can see clearly. For a normal eye, this distance is infinite, that is we can see objects that are infinitely far away.

Hence the far point of the eye is infinity whereas the near point of the eye is 25 cm.
