Complete answer:
A left-wing nationalist political party in India is the All India Forward Bloc (AIFB). In 1939, headed by Subhas Chandra Bose, it emerged as a party within the Indian National Congress. After India's independence, the party re-established itself as an independent political party. In West Bengal, it has its principal stronghold. The new Secretary-General of the party is Debabrata Biswas. In independent India, Sarat Chandra Bose and Chitta Basu, senior Indian leaders, were the party's stalwarts.

The Forward Bloc held its 1st All India Conference in Nagpur the next year, on 20-22 June 1940. The conference proclaimed a socialist political party to be the Forward Bloc. A resolution entitled 'All Power to the Indian People' was passed by the meeting, encouraging militant action to fight British colonial rule. As president of the party, Subhash Chandra Bose was elected and H.V. Kamath as the secretary general.

Let us look at the given alternatives:

1. Even though the party in Jharkhand has a unit, is now facing a split. Option 1 is therefore incorrect.
2. The State Forward Bloc is located in Orissa, but is based in Bengal. Option 2 is therefore incorrect.
3. A regional party in West Bengal is the State Forward Bloc or the All India Forward Bloc. In 1939, headed by Subhash Chandra Bose, the party emerged as the Indian National Congress faction. Thus, the correct answer is Choice 3.
4. The party had a major presence in Andhra Pradesh during the 1950s, but then it declined sharply. In 2005, the party took the initiative to revive its State Committee for Andhra Pradesh. Option 4 is therefore not the correct answer.

Therefore the answer is Option 3.

Note:
At a rally in Kolkata, the founding of the Forward Bloc was revealed to the public.
After a period of 65 years, the All India Forward Bloc(AIFB) elected a new state secretary. The position was vacant following the death of Ashok Ghosh, who had been Secretary of State for the Forward Bloc since 1952.
The party won 0.4% of the votes and three seats in the 2004 Lok Sabha elections (All from West Bengal).
